- چکیده
- چکیدههدف پژوهش: کمردرد مزمن غیر اختصاصی (CNSLBP) از اختلالات شایع اسکلتی-عضلانی است که با درد، اختلال حس عمقی، تعادل و کنترل حرکتی همراه است. با وجود روشهای درمانی متعدد، شواهد کافی از اثربخشی آنها محدود است. تمرینات ترکیبی و DNS بهعنوان روشهای مؤثر مطرح شدهاند، اما مقایسه مستقیم اثر آنها در مردان غیرورزشکار اندک است. هدف این پژوهش، مقایسه اثر تمرینات ترکیبی و DNS بر درد، حس عمقی، تعادل، استقامت و کنترل حرکتی در مردان مبتلا به CNSLBP است.روش شناسی پژوهش: مطالعه به روش نیمه تجربی با طرح پیشآزمون و پسآزمون و گروه کنترل اجرا شد. ?? مرد مبتلا به کمردرد مزمنغیر اختصاصی به طور تصادفی در سه گروه ?? نفره شامل تمرینات ترکیبی، تمرینات DNS و گروه کنترل تقسیم شدند. مداخلات به مدت ? هفته و در ? جلسه در هفته انجام شد. دادهها شامل شاخصهای درد، حس عمقی، تعادل ایستا و پویا، استقامت عضلانی و توانایی کنترل حرکتی بود. تحلیل دادهها با آزمون شاپیرو-ویلک و تحلیل کوواریانس (ANCOVA) همراه با آزمون تعقیبی بونفرونی انجام شد.یافتهها: نتایج نشان داد که هر دو نوع تمرین باعث کاهش معنادار درد و بهبود حس عمقی و توانایی کنترل حرکتی نسبت به گروه کنترل شدند. تمرینات ترکیبی نسبت به DNS تأثیر بیشتری بر استقامت عضلانی داشت. هر دو مداخله بهبود قابل توجهی در تعادل ایستا ایجاد کردند، اما در تعادل پویا تفاوت آماری قابل توجهی مشاهده نشد.نتیجهگیری: تمرینات ترکیبی و DNS به عنوان مداخلات مؤثر برای کاهش درد و بهبود عملکرد حرکتی در مردان مبتلا به کمردرد مزمن غیراختصاصی معرفی میشوند، با این تفاوت که تمرینات ترکیبی اثر بیشتری بر استقامت عضلانی دارد. یافتهها تأکید بر اهمیت طراحی برنامههای تمرینی هدفمند در توانبخشی کمردرد مزمن غیر اختصاصی دارند. کلیدواژهها: کمردرد مزمن غیر اختصاصی،تمرینات ترکیبی، DNS، درد، حس عمقی، تعادل، استقامت عضلانی، توانایی کنترل حرکتی
- Abstract
- Abstract Objective: Non-specific chronic low back pain (NSCLBP) is a common musculoskeletal disorder associated with pain, impaired proprioception, balance, and motor control. Despite the availability of various therapeutic approaches, evidence regarding their effectiveness remains limited. Combined exercises and dynamic neuromuscular stabilization (DNS) have been proposed as effective interventions, but direct comparisons of their effects in non-athletic men are scarce. This study aimed to compare the effects of combined exercises and DNS on pain, proprioception, balance, muscular endurance, and motor control in men with NSCLBP. Methods: This semi-experimental study employed a pretest-posttest design with a control group. Thirty-three men with NSCLBP were randomly assigned to three groups (n=۱۱ each): combined exercise, DNS, and control. Interventions were conducted over ۸ weeks, with two sessions per week. Outcome measures included pain, proprioception, static and dynamic balance, muscular endurance, and motor control. Data were analyzed using the Shapiro-Wilk test and analysis of covariance (ANCOVA) followed by Bonferroni post hoc tests. Results: Both interventions significantly reduced pain and improved proprioception and motor control compared to the control group. Combined exercises showed greater effects on muscular endurance than DNS. Both interventions significantly improved static balance, whereas no significant differences were observed in dynamic balance. Conclusion: Combined exercises and DNS are effective interventions for reducing pain and enhancing motor function in men with NSCLBP, with combined exercises demonstrating superior effects on muscular endurance. These findings highlight the importance of designing targeted exercise programs in the rehabilitation of chronic low back pain.
