بررسی ارتباط بین حداکثر زاویه والگوس داینامیک زانو حین تکالیف حرکتی با تقاضای عملکردی در ورزشکاران تفریحی مرد جوان

    چکیده

    هدف این پژوهش بررسی ارتباط بین حداکثر زاویه والگوس داینامیک زانو (DKV) طی سه تکلیف حرکتی اسکوات تک‌پا (SLS)، فرود تک‌پا (SLL) و پرش و فرود دوپا (DVJ) با مجموعه‌ای از پارامترهای عملکردی شامل کنترل تنه، قدرت عضلات هیپ و زانو، تعادل پاسچرال و دامنه دورسی‌فلکشن مچ پا در ورزشکاران تفریحی مرد جوان بود. پژوهش حاضر توصیفی–همبستگی و بر روی ۲۸ آزمودنی ۱۸ تا ۲۵ ساله انجام شد. داده‌های کینماتیکی با تحلیل ویدئویی دوبعدی نرم‌افزار Kinovea و داده‌های عملکردی با آزمون‌های استاندارد از جمله دینامومتر دستی و آزمون تعادل Y و آزمون لانچ جمع‌آوری گردید. تحلیل آماری شامل آزمون‌های همبستگی پیرسون و اسپیرمن، تحلیل واریانس با اندازه‌گیری‌های مکرر یا فریدمن و رگرسیون چندگانه بود.

    نتایج نشان داد که میانگین زاویه والگوس در تکلیف SLL بیشترین و در DVJ کمترین بود و اختلاف میانگین‌ها بین برخی تکالیف معنادار بود. در بررسی ارتباطات، در تکلیف SLL گشتاور فلکشن و اکستنشن تنه، گشتاور اکسترنال روتیشن هیپ و نمره آزمون Y-Balance با DKV همبستگی معنادار داشتند. در DVJ تنها دامنه دورسی‌فلکشن مچ پا با DKV رابطه منفی معنادار نشان داد. در SLS هیچ ارتباط معناداری مشاهده نشد. نتایج رگرسیون چندگانه نیز بیانگر آن بود که هیچ متغیری به تنهایی قادر به پیش‌بینی تغییرات DKV در تمامی تکالیف نبود.

    به طور کلی یافته‌ها بیانگر ماهیت چندعاملی (multifactorial) DKV است که تحت تأثیر تعامل پیچیده عوامل بیومکانیکی و عصبی–عضلانی مختلف قرار دارد. این نتایج اهمیت توجه هم‌زمان به کنترل تنه، عملکرد عضلات هیپ، قدرت زانو، دامنه حرکتی مچ پا و تعادل پاسچرال را در طراحی تمرینات پیشگیرانه و توانبخشی نشان می‌دهد. همچنین تأکید می‌شود که برای غربالگری و پیشگیری از آسیب‌های زانو، استفاده از تکالیف با تقاضای عملکردی بالاتر مانند SLL می‌تواند حساسیت بیشتری در شناسایی نقص‌های عملکردی داشته باشد.

      

    Abstract

      The aim of this study was to investigate the relationship between dynamic knee valgus angle (DKV) during three motor tasks: single-leg squat (SLS), single-leg landing (SLL), and double-leg jumps and landings (DVJ) with a recreational set of functions including trunk control, hip and knee strength, postural function, and dorsiflexor range of motion. The present descriptive-correlation study was conducted on ۲۸ subjects aged ۱۸ to ۲۵ years. Kinematic data were collected with ۲D video analysis using Kinovea software, and functional data were collected with standard tests including hand-held dynamometer, comparison test, and launch test. Statistical analysis included Pearson and Spearman correlation tests, analysis of variance with Muir or Friedman measurements, and multiple regression. The results showed that the valgus angle was lower in the SLL task and the lowest in the DVJ task, and the difference between the differences between some tasks was significant. In examining the relationships, in the SLL task, trunk flexion and extension torque, hip external rotation torque, and Y-Balance test score were significantly correlated with DKV. In DVJ, only ankle dorsiflexion range showed a significant relationship with DKV. In SLS, no relationship was observed. The results of multiple regression also indicated that no change alone was able to predict DKV change in all tasks. Overall, the findings indicate a multifactorial DKV that is influenced by multiple biomechanical and neuromuscular factors. These important results indicate the simultaneous attention to control, hip function, knee strength, ankle and inter-postural range of motion in the design of preventive and rehabilitation exercises. Also, tasks using higher performance such as SLL can be used for screening and injury, which can be better in identifying functional functions.
